Documents to be submitted by
First-Time Passport Applicants Born in the United States
BY APPOINTMENT ONLY
· Original U.S. birth certificate: (original and 2 copies). Abstracts or mutilated certificates cannot be accepted. If the applicant has dual citizenship, please submit Mexican birth certificate, (original and 2 copies).
· Current photo identification of the applicant: (original and 2 copies).
· Current photo identification of both parents: (original and 2 copies).
· The applicant must be present. If the applicant is under 18 years of age, at least one parent must be present along with the applicant. If the applicant is under 16 years of age, both parents must be present: If both parents are not present for the appointment, it is necessary to bring the notarized written consent (in English) of parent not present for minor child’s passport application (child under 16 years of age), along with a copy of absent parent’s identification showing photo & signature.
· Sole custody: Proof that the parent executing the application has sole custody of child may also be accepted (i.e. birth certificate listing the mother as the only named parent, court order granting sole legal custody to parent, death certificate to deceased parent).
· Prenatal documents: medical records, medical bills and ultrasounds.
· Documents from the US hospital where the applicant was born (wrist bands, crib card, photos at the hospital, discharge orders).
· Photos of pregnancy, birth photos taken in the hospital before and after the birth.
· Photos of the applicant from birth to present.
· Family photos taken during their stay in the United States.
· Proof of parents’ presence in the United States around time of birth of the applicant: Job documents, earning statements, phone bills, tax returns, parents marriage certificate, family photos in the U.S., personal letters, birth certificates of siblings born in the United States, parents’ drivers licenses or State ID cards, census letter.
· Migratory document given when first entered to Mexico: Travel consent signed by parent, if applicable.
· School records: Transcripts, report cards, diplomas from elementary, high school, and university, school yearbooks, or class photos, current or expired ID’s.
· 2 color photos of the applicant 2 x 2 inches = 5 x 5 cms
· Please bring the FORM DS-11 (Application for a US Passport)
with you to your appointment.

· Application fee for a US Passport
Minor (under 16 years of age): $85.00 USD or $1,232.50 pesos
Adult: $100.00 USD or $1,450.00 pesos

· In certain cases, documents in addition to those listed herein may be required.
· The Passport application process takes about 8 to 10 weeks.
· Our office in San Miguel de Allende does not have copying or photo services.
