Documents to be submitted to obtain a
Consular Report of Birth Abroad (CRBA)
for a Child Born in Mexico to U.S. Citizen Parents
BY APPOINTMENT ONLY
· Original or certified copy of the Mexican Birth Certificate: (original and 2 copies). A provisional copy cannot be accepted.
· Proof of parent’s citizenship: (original and 2 copies). Please show one of the following documents for one or both parents: US Passport (valid or expired); original U.S. Birth Certificate; or original Certificate of Naturalization.
· Proof of both parents’ identity: (original and 2 copies). Driver’s license, state ID card, school ID, Mexican voting card, passport.
· If only the child’s father is a US citizen, proof of parents’ relationship: (original and 2 copies). If parents were married prior to the date of conception, please provide an original or certified copy of the marriage certificate and divorce decree (if applicable). If parents were not married at the time of conception, please provide proof of the existence of the relationship at that time (personal letter and cards, dated photographs, telephone bills).
· Prenatal documents: medical records, medical bills and ultrasounds.
· Documents from the U.S. hospital where the applicant was born (identification bracelet, wrist bands, crib card, photos at the hospital, discharge orders, hospital bill).
· Photos of pregnancy, birth photos taken in the hospital before and after the birth.
· Photos of the applicant from birth to present.
· Proof of parents’ presence in Mexico: (original and 2 copies). INM tourist card, FM-2, FM-3, Mexican passport.
· If only one parent is a U.S. citizen, proof of parents’ physical presence in the U.S. prior to the birth of the child: (original and 1 copy). The parent must prove at least 5 years of physical presence in the U.S., at least 2 of which are after the age of 14. The period of physical presence need not be continuous. Presence must be proven with concrete evidence (school records, tax forms with W-2, earnings statement, pay receipts, passport stamps). Please note that if the child was born before Nov. 14, 1986, the parent must prove 10 years physical presence, at least 5 after the age of 14. You are encouraged to consult with your Consular Officer to discuss these requirements and the required documentary evidence.
· Please bring the FORM DS-2029 with you to your appointment. You may fill it out in the computer or by hand.

$65.00 USD or $942.50 pesos (please bring cash and the exact amount)
· Some parents choose to apply for a CRBA and passport for their child at the same time. Requirements for a US Passport: 2 color photos of the applicant 2 x 2 inches = 5 x 5 cms

Minor (under 16 years of age): $85.00 USD or $1,232.50 pesos

· Both parents and baby are required to appear at the time of the application.
· Our office in San Miguel de Allende does not have copying or photo services.
